A college student wakes up after a night of heavy drinking and decides to head back out to the bar. Unfortunately, they catch the eye of one of three very undesirable gentlemen. Strade is a psychopathic serial killer, Sano is a sadistic scientist, and Rire is a charming and powerful demon. The protagonist finds themselves at the mercy of one of these men, and must attempt to survive whatever torture is inflicted upon them.

Boyfriend to Death is an erotic visual novel featuring several extreme fetishes under the general umbrella of guro. Most of the sex scenes are non-consensual, and the endings typically involve the protagonist's agonizing death.
