The game takes place in 1993, when a group of five students and members of an occult club travel to Silent Hill to solve the mystery of Toluca Lake. It is known that seventy-five years ago a boat called "Little Baroness" disappeared with all the crew and passengers on the lake, and the bodies have never been found. The next morning after the arrival, students Eric and Tina (the protagonists) discover that one of their club members is injured while others are missing, and that the whole town is crawling with monsters.

Silent Hill: The Arcade is a rail shooter in the vein of The House of the Dead games, complete with off-screen reloading and weapon power-ups. The players will shoot down hordes of monsters in various familiar (and not so) locations including Brookhaven Hospital, Silent Hill Historical Society, Lakeside Amusement Park etc., on their way to the heart of the catastrophe — a little girl aboard the Little Baroness. The game features four endings, one them being a traditional "UFO" ending.
